Leaves of absence are managed by the Benefits area in the Human Resources Department. This includes medical leaves, parental leaves, personal leaves, military leaves, and sabatticals. A Leave of Absence and Hold Request Form must be completed by the supervisor and forwarded to Human Resources at the West campus before a leave of absence can be officially initiated. A leave of absence status change form must be used to communicate any changes, to include return to work, when changes occur.

COBRA
The Consolidated Omnibus Reconciliation Act (COBRA) provides benefits extension options to employees when a status
change results in a loss of benefits to the employee and/or their covered dependents. This applies to employment
status changes such as resignation or an employee's status changing from benefits eligible to not benefits eligible
due to a regular schedule change. This also applies to certain life events such as divorce, death, and when a
dependent child is no longer covered due to their age. Please contact the ASU West Campus Human Resources
office immediately if this situation occurs.

Benefits-eligible employees are granted time off
from work with pay for ten holidays designated by the University.
Holiday pay is based on an eight-hour work day. Eligible employees
who are less than 100 percent FTE are granted time off with pay for
a pro-rated portion of the holiday based on their percentage of FTE.

Benefits eligible employees earn vacation time based upon their length of employment at Arizona State University.
Accrual rates are shown in the table below. Benefits eligible employees less than 100% FTE will have their vacation
accrual pro-rated accordingly.
Sick leave is accrued by all benefits-eligible employees. Employees working 100% FTE accrue 4 hours each pay
period. Employees working less than 100% FTE but at least 50 percent FTE are credited with sick leave adjusted
to the percentage of time worked. Eligible faculty accrue sick leave during the academic year only. Sick leave
